Knowing what a visit costs before you book is reasonable. Here is what we can tell you.
In-network providers
We are an in-network provider with Blue Cross Blue Shield, Sanford Health Plan (except Sanford Health employee plan), Medicare, HealthPartners, TriCare West, Medica, and ND WSI. We are always working to credential with additional insurance providers to become in-network providers.

What to ask your insurer
- Is Connect Physical Therapy in-network for outpatient physical therapy?
- What is my deductible, and how much of it have I met?
- Do I have a per-visit copay or coinsurance?
- Is there a visit limit on my plan this year?
- Does my plan require a referral even though North Dakota does not?
